About Internal Medicine

Veterinary internal medicine specialists are dedicated to the thoughtful evaluation and management of complex, multi-system diseases, including conditions affecting the gastrointestinal tract, liver, pancreas, and urinary tract, kidneys, lungs, blood, endocrine system, and immune system, as well as infectious and immune-mediated disorders. Pets are referred to an internist when clinical concerns are nuanced, diagnoses require deeper investigation, or when standard treatments have not led to improvement. Our Internal Medicine team works in close collaboration with your primary care veterinarian, extending their care through advanced diagnostics, specialized expertise, and coordinated access to additional specialty services when needed.

In keeping with our commitment to exceptional medical care and thoughtful service, our Internal Medicine department is led by our board-certified specialist Dr De Witte and supported by highly trained veterinary nurses and client liaisons. We combine the precision of a specialty practice with the attentiveness of a concierge experience, delivering meticulous diagnostics and individualized treatment plans tailored to each patient. Clear, compassionate communication is central to our approach, we take the time to listen, explain, and guide you through every step of your pet’s diagnosis and care. In close partnership with your referring veterinarian, we work collaboratively to ensure continuity, clarity, and the best possible outcome for your pet.

Gastroenterology

Chronic enteropathies (Chronic gastrointestinal disease), with clinical signs such as chronic recurrent diarrhea, vomiting, regurgitation, and weight loss. Can include inflammatory conditions such as inflammatory bowel disease (IBD), cancerous co nditions (such as small cell lymphoma in cats), or infectious (Boxer colitis, fungal diseases, etc)
Pancreatitis, Exocrine pancreatic insufficiency (EPI)

Endocrinology

Pancreas: Diabetes mellitus, Insulinoma, Diabetes insipidus
The adrenal and pituitary glands: Hyperadrenocorticism (Cushing’s disease), Hypoadrenocorticism (Addison’s disease), Hyperaldosteronism, Hypersomatropism

Nephrology

Chronic and acute kidney disease

Our Internal Medicine team offers a wide range of sophisticated, minimally invasive diagnostics and procedures, delivered with precision and discretion: terms and share the next steps or recommendations with you and your veterinarian.

Endoscopy

Gastrointestinal diseases, such as chronic enteropathies (such as inflammatory bowel disease or cancer), foreign body removal, and esophageal balloon procedures
Respiratory diseases, by performing respiratory sample collection with a tracheal wash, bronchoalveolar lavage and biopsies when needed
Urogenital diseases, to collect biopsies, investigate structures, urinary stones, and ectopic ureters. Cystoscopy and urethroscopy services are offered.

Advanced imaging and related procedures

Ultrasound:

- Abdominal, Thoracic and small parts (parathyroid glands, thyroid glands, ocular, musculo-skeletal)
- Ultrasound-guided needle aspirate and biopsies
- Ultrasound can be performed either:
  - During consultations
  - As an out-patient ultrasound (OPUS), no consultation. Findings are relayed to the primary veterinarian, who will discuss results with you.

Computer Tomography (CT) scan, to investigate:

- Liver shunts
- Ectopic ureters
- Masses or abscesses
- Respiratory or abdominal diseases

Cavity fluid sampling and fluid removal

Thoracocentesis (fluid removal from the chest)
Abdominocentesis (the abdomen)
Arthrocentesis (the joints)

What is the schedule?

Clinic hours: The Internal Medicine Service at URVet is open for appointments and procedures Tuesday through Thursday, 8:00 am to 6:00 pm. An Internal Medicine staff is available during limited hours on Monday and Friday to address questions and prescription refills.
Internal Medicine appointments: Scheduled between 9:00 am and 4:00 pm. This allows us to provide the highest level of medical care and dedicate adequate time to discuss findings and develop thoughtful treatment plans. Same-day appointments and procedures are accommodated whenever possible.
Scheduled procedures (endoscopy, CT scan, blood glucose curves, etc.): Please plan to drop your pet off around 8:00 am. Our service coordinator will contact you in advance to confirm exact timing and details. URVet is a 24-hour hospital, so you may drop off earlier or pick up later than Internal Medicine hours if needed
Outpatient Ultrasound (OPUS): Scheduled between 9:00 am and 5:00 pm. OPUS is intended for stable patients only and involves ultrasound imaging alone. Results are shared directly with your primary veterinarian, who will review them with you.
Drop-off appointments: We can facilitate drop-off appointments for families who need to be at work. Dr. De Witte will contact you by phone to review results and recommendations (drop-off around 8:00 am, pick-up by 6:00 pm). URVet is a 24-hour hospital, so you may drop off earlier or pick up later than Internal Medicine hours if needed
In-patient care: The URVet Internal Medicine Service accepts Internal Medicine cases transferred from the ER or directly referred by your primary veterinarian

What Should I Bring to My Appointment?

Medical records:
- When you schedule your appointment, we will email you an Internal Medicine questionnaire. Please return the completed questionnaire and all prior medical records as early as possible, ideally the day you make the appointment or at least one week before your visit, and no later than the day before
- This allows our team to review your pet’s history in advance, become familiar with the case, and be fully prepared for your visit.
- Please include any new test results from any recent primary veterinarian you have seen.
Fasting instructions: Most patients should arrive fasted (no food for 12 hours; water is allowed), unless your pet is diabetic or under 4 months of age.
- Fasting is important for accurate ultrasound imaging and certain blood tests.
- Our client coordinator will guide you if you have any questions or special circumstances.

Will My Primary Veterinarian Be Involved in My Pet’s Care?

Absolutely. After your appointment, we provide your primary veterinarian with a detailed consultation report and recommendations.
For more complex cases, your primary veterinarian may ask us to manage care until your pet is stable. We value close collaboration with our referral community and work together in a partnered approach focused on your pet’s best outcome.

Will Other Veterinary Specialists Be Involved in My Pet’s Care?

Possibly. Every patient is unique, and in some cases additional specialists, such as surgeons, cardiologists, neurologists, radiologists, or other internists, may need to be consulted before, during, or after your pet’s treatment.
If we believe your pet would benefit from additional expertise, this will be discussed with you during your visit.

How Much Does It Cost?

- We understand that finances are an important part of medical decision-making. Costs can vary significantly depending on your pet’s condition and whether diagnostic tests or procedures are needed (such as endoscopy, CT, biopsies, or specialized lab work).
- We do not provide procedure cost estimates or schedule specific tests before an Internal Medicine consultation. The initial consultation offers the greatest value: our specialist reviews your pet’s history, performs an exam, and determines the most appropriate diagnostics. As in human medicine, the primary or general practitioner might have discussed with you’re the anticipated tests, but the specialist does not proceed without first evaluating the individual case. We consider your pet’s stability, comorbidities, and medical history, explain what potential tests mean, discuss financial implications, and then recommend the most appropriate diagnostic plan.
- You will receive an itemized estimate for recommended diagnostic and treatment options at the time of, or shortly after, your visit. Payment is due at the time services are provided.
- Pet insurance is often very helpful in covering a portion of medical costs. We are happy to work with your insurance provider to facilitate reimbursement following your visit.

- If your pet is not insured, while we do not offer in-house payment plans, third-party options such as CareCredit may help with cost management. Please contact us directly for more information.

How Does an Internal Medicine Drop-Off Appointment Work?

Drop-off patients should plan to arrive at URVet ideally between 7:30 am and 9:00 am. URVet is a 24-hour hospital, so you may drop off earlier or pick up later than Internal Medicine hours if needed

Discharge may occur throughout the day once treatments are completed; however, we are happy to hold pets until the end of the day if needed. The Internal Medicine service closes at 6:00 pm, and all patients must be discharged by 6:00 pm. URVet is a 24-hour hospital, so you may drop off earlier or pick up later than Internal Medicine hours if needed
When special accommodations are needed, mid-day drop-off may occasionally be arranged on a case-by-case basis.

- - At each visit, we will present our recommended diagnostic and treatment plan, along with any reasonable alternative options and the expected outcomes of each.
  - We recognize that there is a large amount of information available online, not all of it accurate. Our role as veterinary specialists is to provide up-to-date, evidence-based recommendations, informed by years of clinical experience.
  - We welcome and encourage thoughtful questions and discussions and are always happy to talk through your pet’s care in detail.
  - All of the client calls and emails are triage first by our Internal Medicine nurses, that have a great deak of medical nowled and theyc an answer the vast amount of questions, and they are trained to do client calls. Please let them do their job! We are here to help you, and we all work as a team.

Primary contact person

- - All client calls and emails are first triaged by our Internal Medicine nursing team. Our nurses have extensive medical knowledge, are specifically trained for client communication, and can answer the vast majority of questions. Please allow them to do their work—they are here to help you, and we collaborate as a unified team to support you and your pet.

- The phone number and email address provided when scheduling your appointment or admitting your pet will be designated as the primary point of contact. The Internal Medicine team will communicate medical updates and discuss medical and financial decisions with this person directly.

- To ensure clear and efficient communication, the primary contact is responsible for sharing information with other family members. Please confirm that we have your correct email address, as follow-up results are typically communicated electronically.

- Please allow 24 to 48 hours to receive written consultation notes following your Internal Medicine visit.

- Email is monitored regularly during the Internal Medicine Service Schedule (Tuesday through Thursday, 8:00 am to 6:00 pm, and limited hours on Monday and Friday. Th email is not checked during the week end). However, urgent concerns should not be communicated via email. Please allow 24–48 hours for a response, depending on message volume.
Emergencies
- If your pet is experiencing an emergency or appears to be in crisis, please proceed immediately to our URVet Emergency service or if needed your nearest emergency veterinary facility.
